Registering with the surgery
To register with the surgery you need to complete three forms, these form can be collected at reception or downloaded here:
- New Patient Registration Form
- Patient Questionnaire (DOCX, 20KB)
- NHS Summary Care Record Consent Form
When you register, it’s helpful to have your NHS number and your ID. You can use the NHS website to find your NHS number.
You can also register online using the GP Online Registration Service.
Please ensure when you leave your current practice, you have a sufficient amount of medication for yourself as we will not be able to issue you this until you are registered and you would need to contact NHS 111.
